Town Clerk - Birth & Death Certificates

  • The Town Clerk’s Office, located in Town Hall in Lindenhurst, is the official keeper of birth information for those births which occurred in the town of Babylon. Please have proper identification to verify you are the person of record. Records for the births outside of the Town of Babylon are not kept in Town Hall. For these records, please contact the town or city the birth occurred in. If you have any questions, please contact the Town Clerk’s Office at (631) 957-3007. Records for births outside of the Town of Babylon are not kept in Town Hall. For these records, please contact the town or city the birth occurred in.

  • Note: The Town of Babylon Clerk’s office will only have a copy of your Birth Certificate if you were born in the Town of Babylon. See attached list of Towns and Villages. If you were born in Brunswick Hospital, please contact the Village of Amityville at 631-264-6000. Only the person named on the Birth Certificate if 18 years of age or older, the parents listed on the Birth Certificate or a lawful representative of the person named or the parents of the person named on the Birth Certificate. 

    A. In order to obtain a certified copy of your birth certificate by mail, you will need:

    1. Fill out the attached form: (Birth Certificate Request Application)
    2. A notarized letter requesting a copy of your birth certificate which must include both your parent’s names with your mother’s maiden name and your date of birth.
    3. Copy of your driver’s license or non-driver’s license
    4. Money order made out to the Town of Babylon on the amount of $10 per copy.
    5. Mail your request to:

  • The Town Clerk, located in Town Hall, is the official keeper of all death records for the town. For more information on death records, please contact the Town Clerk’s Office at (631) 957-3007.

  • Note: The Town of Babylon Clerk’s Office will have a copy of the Death record if the person was pronounced deceased in the Town of Babylon. If the person was pronounced deceased in a hospital, please call the Town in which the hospital resides. See attached list of Towns and Villages.

    In order to obtain a certified copy of a Death Certificate by mail, you will need to complete and mail:

    1. The attached Death Certificate Request form; (Death Request application form) send proof of relationship to the deceased which be either a copy of your birth certificate or copy of your marriage certificate. If neither of these are not an option please call 631-957-3007 for additional instruction
    2. Copy of your driver’s license
    3. Money order made out to the Town of Babylon in the amount of $10.00 for each requested copy
    4. Mail your request to :

    Note: Death certificates can be obtained by the decedent’s spouse, parent, child or sibling. The requester must provide either a VALID driver’s license, non-driver’s ID or passport and provide proof of relationship (spouse to provide a Marriage Certificate only; parent, child or sibling to provide a Birth Certificate only).  If you have any questions, please contact the Town Clerk’s office at (631) 957-3007.
